If you would like to be invited to join our patient portal, where you can fill out registration paperwork online, and communicate with Pat, please fill out the form below. After you have completed the form, please text (605)202-7996 and let us know you have requested a Patient Ally account. At that point, we can add you as a new patient. You will receive an email from Patient Ally, which will confirm your email, and then your phone number in the process. The email will link you to setting up an account. The Patient Ally portal will allow you to request appointments online, AFTER your first appointment with Pat. The first appointment must be set directly with Pat, either through email, text or phone.
